Utah woman claims vet mistakenly euthanized pet dog after calling wrong family

A Utah family is coping with losing their dog after a vet reportedly euthanized him by mistake.

Andrea Martinez took to Facebook to share the devastating story on Monday, writing that the family took their pet dachshund Ziggy to the emergency room after “his breathing got weird.”

The emergency vet recommended surgery for Ziggy, which the family agreed to, Martinez said.

While the pup was undergoing the invasive procedure, the vet realized it was going to be “a lot more work and money than they expected” and called to get permission from the family to continue with the surgery or “go ahead and lay him down.”

However, Martinez claims she never received that call.

“But they didn’t call me,” Martinez shared in the post. “[T]hey called another [dog’s] mom thinking it was me, and she said no to go ahead and lay him down.”

Martinez did not find out her dog had been euthanized until someone from the vet’s office called later to tell her Ziggy had been put down. She said that when she “freaked out,” the vet realized “they called the wrong person the first time.”

“Then he got back on [the phone] and he apologized and said, ‘I’m so sorry that this happened, we got confused and ended up calling another dog’s mom,’” Martinez told KUTV. According to the vet, there was another dog named Ziggy, and the vet initially contacted that Ziggy’s family.

According to Martinez, she said she would have never agreed to let her dog go “without trying more” and blames the vet for “rob[bing] me the choice of saying ‘at least we tried.’”

Since finding out the heartbreaking news, Martinez has said she “can’t stop crying” and doesn’t know how to break it to her 6-year-old that “her best friend died.”

“Ziggy followed me EVERYWHERE and cuddled me every single night. He made my anxiety bearable. I feel so sick that this happened,” she wrote on Facebook.

The name of the vet in Davis County was not shared, but according to KUTV, they apologized for the unfortunate incident and waived the bill, as well as gifted the family an urn, plaque and Christmas ornament with Ziggy’s paw print.

Though with Ziggy’s condition during the surgery, there is no guarantee he would have made it through if they continued trying, per KUTV. However, that does little to help Martinez cope.

“This whole thing seriously makes me so sick to my stomach,” she said.

Utah boy, 6, dies after falling off snow blower

A 6-year-old boy in Utah died after he fell from a snow blower, according to the Utah County Sheriff’s Office.

Fox 13 reported that Herman Slaughter was helping his father and 9-year-old brother at Aspen Grove Family Camp in Provo Canyon when he fell from a Kubota Skid Steer loader.

He sustained critical injuries after a piece of equipment struck his head, according to the Utah County Sheriff’s Office.

The sheriff’s office said the snow remover “bounced or lurched,” and the boy fell forward out of the seating area “into the path” of the machine.

Herman was pronounced dead at the scene, the sheriff’s office said.

Fall during Delicate Arch hike in Utah kills 2, injures 1

A hike to Utah’s iconic Delicate Arch turned tragic when three hikers fell and two of them were killed.

The arch sits on the edge of a sandstone bowl with steep slopes, and authorities said the three fell into that bowl at about 7:30 a.m. Friday.

“It appears they were all from California and that they may be related,” the Grand County Sheriff’s Office said in a news release. “Authorities are currently investigating and more information will be released as it becomes available.”

The victims were a 65-year-old man and a 60-year-old woman, the news release said.

The survivor was a 30-year-old man. He was taken to the hospital by helicopter.

Chief Ranger Scott Brown told KSL-TV the cause of the fall was not known, though the trail was wet and slick at the time of the incident.

Rangers closed Delicate Arch Trail temporarily following the incident but reopened later that evening, The Washington Post reported. Park officials said in a tweet that winter conditions have caused slippery hiking and warned visitors to proceed with caution.

“People come from all over the world to visit Arches National Park, and visiting Delicate Arch is on the top of many visitors’ to-do lists,” the National Park Service says on its website.

The trail to see Delicate Arch up close and personal is 3 miles roundtrip and climbs 480 feet, according to the park service.

At the end of the trail hikers follow a narrow rock ledge with exposure to heights.

“Rangers frequently rescue people who underestimated the trail’s difficulty,” the park service says.

Man’s body found in freezer of dead Utah woman was that of her husband, authorities say

A Utah man whose body is believed to have been stored in a freezer for as much as 11 years was the husband of a woman who’d recently died in their apartment, authorities said Tuesday.

The body of Paul Edwards Mathers, 69, was discovered on Nov. 22 after officers in the Salt Lake City suburb of Tooele found his wife, Jeanne Souron-Mathers, 75, dead on a bed in their unit at the Remington Park Apartments.

Investigators believe Souron-Mathers was aware of the body in her freezer, said Tooele Police Sgt. Jeremy Hansen.

“That’s why we strongly suspect foul play,” he said, according to KSL-TV.

It was not clear if authorities suspect Souron-Mathers was involved in her husband’s death.

Officers were sent to the home because she hadn’t been seen in two weeks. Her death is not considered suspicious.

Investigators estimate Paul Mathers’ body had been inside the freezer for as long as 11 years, based on when residents at the apartment complex last saw him.

“When we started talking to different people in different apartments that surround hers, they all recall seeing a guy there, but everyone’s time frame was different,” Hansen said.

An autopsy was not able to narrow down a time frame or how he died. Souron-Mathers had lived in the apartment since 2007.

Utah Jazz arena evacuated minutes after game over suspicious shoebox

Vivant Smart Home Arena in Salt Lake City was evacuated Friday night after a suspicious shoebox was found underneath a table in an arena restaurant, according to reports.

The discovery came just minutes after the Utah Jazz defeated the Golden State Warriors 113-109 in an NBA game.

Players still in uniform left the building with the crowd, without even going to their locker rooms. Most of the spectators had already exited before the evacuation started.

“We don’t know if it’s anything explosive. So far what we have is just a suspicious package,” Salt Lake City Police Department Lt. Carlos Valencia told reporters after the evacuation. “Public safety is paramount to us and we take these complaints very seriously.”

A police bomb squad was called in after a bomb-sniffing dog reacted to the shoebox, Valencia said.

Salt Lake City police said the package turned out to be a toolbox and the teams were allowed to return to the building about two hours later, the Jazz said.

No injuries were reported.

Separately, about 25 minutes after the evacuation, three people were wounded in a drive-by shooting outside a concert venue called The Complex just two blocks away, The Salt Lake City Tribune reported. Valencia said one of the victims was shot in the chest but was expected to survive. No arrests were reported as of early Sunday.

A former Utah stake president in The Church of Jesus Christ of Latter-day Saints pleaded guilty to wire fraud amid allegations he used his influential position to scam $1.5 million from friends and fellow church members over a 12-year period.

Robert Glen Mouritsen, 72, changed his “not guilty” plea in U.S. District Court in Salt Lake City on Wednesday — pleading guilty to one count of felony wire fraud as part of a plea deal.

Mouritsen had been charged with three counts of wire fraud and three counts of money laundering stemming from the scheme he allegedly launched in 2006, KUTV reported citing a felony indictment.

Over a 12-year period, Mouritsen allegedly raked in $1.5 million, including $326,399.51 from one single victim, the outlet reported.

A stake president oversees groups of congregations. Mouritsen held that influential role in Kaysville from 1989 to 1997, where he used his “position of prominence to induce friends and fellow church members to give him money to further a financial fraud scheme he called ‘The Project,'” court records obtained by the Salt Lake Tribune showed.

He allegedly told investors that “The Project” “involved a series of complicated international transactions” requiring the help of lawyers and bankers.

Mouritsen told investors it was too costly to keep “The Project” “moving forward,” while adding it was “subject to extremely strict confidentiality agreements and he could not disclose many details,” the outlet added.

He was said to have promised high rates of return, but “The Project” reportedly didn’t produce any returns at all.

Mouritsen also “used a significant portion of investor money for his own personal use and benefit,” the indictment claimed.

Two other victims lost amounts totaling roughly $165,000 and $33,000 from the scheme, the indictment showed. It sought a judgment of about $1.5 million, the outlet reported.

The trial was delayed back in August to allow time for Mouritsen to recover after he suffered a heart attack.

Each count of wire fraud would equate to a maximum of 20 years in federal prison, while each of the money-laundering charges could result in up to 10 years in prison.

A Utah Highway Patrol Officer is being hailed as a hero after he risked his life to pull a man off of train tracks moments before a train barreled through the area.

Dashboard camera footage shows the officer bolting out of his vehicle and running into the path of the oncoming train to rescue an unconscious driver from a vehicle that had crashed on the tracks.

“We’ve got to get out of here, we’ve got a train coming! We’ve got a train coming! We’ve got a train coming!” Trooper Ruben Correa can be heard shouting. Just before the train collided with the vehicle, Correa pulled the driver into the grass surrounding the tracks.

No one was injured in the crash, but the vehicle was destroyed.

“I looked to my left and was able to observe the train was coming pretty fast,” Correa told KSL-TV, “anywhere between 50 – 80 miles per hour.”

“At that point, I actually wasn’t really thinking, I was just doing my job, and the main concern was getting him [the driver] out,” he added.

The vehicle had veered off the freeway, crashed through a fence, and came to a stop on the train tracks after the driver suffered a medical condition. Correa had been handling a traffic stop nearby when he responded to a call of a car on the tracks. “Before I knew it, that train hit that vehicle and we were able to make it out alive,” said Correa.

Correa said he didn’t feel the weight of the situation until he watched the train launch the SUV from the tracks. “That’s when I realized, ‘Oh, wow, that was a lot closer than what I would have liked,’” he said.

The incident backed up traffic on Interstate 80 for four hours and caused delays on the Utah Transit Authority’s Frontrunner commuter train service.

The missing Utah woman who police said disappeared voluntarily has been found dead in her car, according to her family.

Erin Valenti, a 33-year-old CEO of Tinker Ventures, a Salt Lake City-based tech company, was discovered “deceased in her vehicle in San Jose” on Saturday. Before she was reported missing, Valenti was reportedly on her way to the San Jose International Airport in California to head home to Utah on Monday.

“Erin has been found. Please call off the search,” her family said in a statement, posted to Facebook by Missing Pieces Network. “We all greatly appreciate the effort and support people have provided. The family will release more information as they are able to.”

In the “Help Find Erin Valenti” Facebook group, the family continued: “Many of you have seen that the search for Erin has been called off. While we were praying for a different outcome, we are so appreciative for the help and support you have given.”

“Please remember Erin as the beautiful, smart, funny woman that she was. I don’t have any details that have not been in the media. I’m sure that Harrison will thank you himself when he is able, but please be patient,” the post read.

OREGON COLLEGE FRESHMAN REPORTED MISSING, FAMILY SAYS ‘THIS IS NOT LIKE HIM AT ALL’

Valenti’s husband, Harrison Weinstein, pleaded online Wednesday for help finding his wife. He said she was last seen in Palo Alto, Calif., on Monday afternoon, “but never returned her rental car or made it on her flight home to Utah.”

“Her phone has been off since Monday night. If you have seen or heard from her please let me know,” her husband wrote. “Any information to her whereabouts is much appreciated and can be directed to me, her family, or the San Jose Police Department.”

On Friday, San Jose Police Sgt. Enrique Garcia told KSTU investigators were “treating this case as a voluntary missing person.” However, Valenti’s family says she seemed to be distraught on the night she disappeared. Weinstein and Valenti’s parents told the station that they spoke to her on the phone Monday and she seemed manic, though she has no history of mental illness.

A San Jose police officer made contact with Valenti by phone Monday night, the East Bay Times reported. Weinstein told the outlet: “The officer said she wasn’t making any sense. They drove around looking for her on Monday night and never found her.”

Valenti’s mother, Whitey Valenti, said she spoke with her daughter “for hours on and off” on Monday night.

“Her thoughts were disconnected. She talked a mile a minute. She’d say I’m coming home for Thanksgiving, then in the next she was saying she’s in the Matrix.”

The family of a missing Utah woman who was last seen Monday say that police in California incorrectly described her disappearance as voluntary.

Family members say Erin Valenti, CEO of Salt-Lake City-based app developer Tinker Ventures, was last seen in Palo Alto, Calif. She was supposedly on her way to the San Jose airport to head home to Salt Lake City.

Valenti’s husband Harrison Weinstein wrote on Facebook that phone activity suggested Valenti was driving in San Jose. She never returned her rental car or made it on her flight back to Utah, and she’s had no phone or credit card activity since Monday.

San Jose police spokesman Sgt. Enrique Garcia told Fox 13 Friday that “at this time, we are treating this case as a voluntary missing person.” However, Valenti’s family says she seemed to be distraught on the night she disappeared. Weinstein and Valenti’s parents told the station that they spoke to her on the phone Monday and she seemed manic, though she has no history of mental illness.

“I get that people have the right to disappear, but that’s not this,” Weinstein told KTVU. “Everything she said she was planning on coming home. She was trying to get to the airport. She’s not thinking clearly.”

“She seemed confused. We said, ‘Did you drink anything?  Did anybody give you anything?’ and she said, ‘No,'” her mother Agnes Valenti said, “But my daughter is so smart and she is such a strong independent lady and this has never happened before.”

Valenti is described as 5-foot-4 with blond hair. She was last seen wearing a white T-shirt and jeans.

Utah police officer survives crash after truck slams into patrol car along interstate

A highway patrol officer in Utah has been released from the hospital after surviving when a truck lost control and struck his car on the side of an interstate over the weekend.

The Utah Highway Patrol said the crash happened shortly after 1:30 p.m. Saturday along Interstate 15 in Box Elder County, located north of Salt Lake City.

Sgt. Brian Nelson was stopped on the side of the road to investigate his third crash of the day along the stretch of the interstate when a white Dodge pickup truck hydroplaned and hit his vehicle.

“If he would have been outside of his car, I think the damages and injuries would’ve been a lot worse,” UHP Cpl. Colton Freckleton told FOX13.

The highway patrol said a medical helicopter was preparing to pick up Nelson, who reported having “serious neck pain,” but was canceled due to heavy rain.

Nelson was transported to an area hospital, where he was checked out and did not have any visible breaks. He was later treated and released and plans to follow up with his own doctor next week after resting, according to police.

“We’re lucky that he was inside his vehicle enough to take that impact,” Freckleton told FOX13.

Before the crash, officials said that Nelson was warning the driver involved in the initial crash to stay in his car in case anyone else lost control on the highway.

Freckleton told FOX13 the woman driving the truck that hit Nelson’s car had seen the accident and moved over to give them space but did not anticipate hydroplaning on the highway.

“We ask that drivers first reduce your speed and then move over if you have an open lane,” he said.

Officials released video of the crash on Sunday to Facebook, which showed the moment the truck the trooper’s vehicle and the other vehicle involved in the initial crash before skidding down the rain-soaked interstate lanes.

“Another vivid reminder of the importance of SLOWING DOWN in inclement weather,” police said.

Freckleton said in his five years with the highway patrol, he’s been hit twice. The most recent was in August when responding to a traffic accident that he described as a “pretty scary experience.”

He told FOX13 that authorities finding that incidents involving drivers hitting patrol vehicles with their lights on is happening more frequently.

“I don’t see that the message is getting clearly across,” he said Sunday. “I feel like it’s getting worse, and I also think another reason this is happening because drivers are driving distracted.”
